Common use

Tiova Rotacap is routinely prescribed for the maintenance treatment of airflow obstruction in patients with COPD. It is not indicated for the relief of acute bronchospasm and should be used as a part of a comprehensive management plan that may include pulmonary rehabilitation, smoking cessation, and other pharmacotherapies. Dosage and direction

The recommended dosage is the inhalation of the contents of one Tiova Rotacap (18 mcg) once daily, at the same time each day, using the Rotahaler device. The capsule should not be swallowed. Instructions for use:

  1. Remove one capsule from the blister strip immediately before use.
  2. Open the Rotahaler by twisting the mouthpiece.
  3. Insert the capsule into the chamber; close the device.
  4. Press the button on the side of the Rotahaler to pierce the capsule.
  5. Breathe out fully, place the mouthpiece between lips, and inhale deeply and steadily.
  6. Hold breath for up to 10 seconds, then exhale slowly.
  7. Check the chamber to ensure the capsule is empty; discard it after use.

Do not exceed the prescribed dose. If a dose is missed, refer to the “Missed dose” section.

Precautions

  • Not for use in the treatment of acute episodes of bronchospasm.
  • Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction.
  • Immediate hypersensitivity reactions, including angioedema, may occur.
  • Patients should be instructed on proper inhaler technique to avoid inadequate dosing.
  • Monitor for signs of worsening COPD or paradoxical bronchospasm.
  • Avoid getting the powder in the eyes, as it may cause blurring of vision or pupil dilation.

Contraindications

Missed dose

If a dose is missed, it should be taken as soon as remembered on the same day. If it is nearly time for the next dose, skip the missed dose and resume the regular schedule. Do not double the dose to make up for a missed one.

Overdose

Overdose may lead to anticholinergic effects such as dry mouth, visual disturbances, tachycardia, and urinary retention. Treatment is supportive and symptomatic. There is no specific antidote. Dialysis is not appropriate due to high protein binding and extensive tissue distribution.

Storage

Store below 30°C in a dry place. Protect from moisture and light. Keep the capsules in the original blister packaging and remove only immediately before use. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
