FML Forte (fluorometholone ophthalmic suspension 0.1%) is a high-potency topical corticosteroid formulated for the management of severe steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe. It provides targeted anti-inflammatory and immunosuppressive action, reducing edema, fibrin deposition, capillary dilation, and phagocytic migration. Common use

FML Forte is indicated for the treatment of steroid-responsive inflammatory conditions of the eye where a higher potency corticosteroid is clinically warranted. Common applications include severe allergic conjunctivitis, vernal keratoconjunctivitis, episcleritis, anterior uveitis, and postoperative inflammation following ocular surgery. It is particularly valuable in cases where inflammation threatens visual acuity or where standard potency steroids have proven inadequate. The formulation is also used in management of corneal injury from chemical burns or trauma when significant inflammation is present.

Dosage and direction

Instill 1 drop into the conjunctival sac of the affected eye(s) two to four times daily. During the initial 24 to 48 hours, the dosing frequency may be increased to every 2 hours if necessary, based on severity of inflammation. Shake the suspension well before each use. To administer, tilt head backward, pull down lower eyelid to form a pouch, instill drops, and close eyes gently. Apply light pressure to the lacrimal sac for 1-2 minutes to minimize systemic absorption. Do not touch dropper tip to any surface to avoid contamination. Treatment duration should be limited to the shortest period necessary to control inflammation, typically not exceeding two weeks without re-evaluation.

Precautions

Prolonged use may result in glaucoma, damage to the optic nerve, defects in visual acuity and fields of vision, posterior subcapsular cataract formation, or may aid in the establishment of secondary ocular infections from fungi or viruses liberated from ocular tissues. Use with extreme caution in patients with a history of herpes simplex keratitis. Intraocular pressure should be routinely monitored even though it may be difficult in children and uncooperative patients. If signs of hypersensitivity develop, discontinue use. Patients should be advised not to wear contact lenses during treatment for inflammatory conditions.

Contraindications

FML Forte is contraindicated in most viral diseases of the cornea and conjunctiva including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella. Also contraindicated in mycobacterial infection of the eye, fungal diseases of ocular structures, and in individuals with known hypersensitivity to any component of this medication. Not for use in purulent untreated bacterial infections without appropriate concomitant antimicrobial therapy.

Possible side effect

Common side effects include transient stinging or burning upon instillation (approximately 15% of patients), blurred vision, foreign body sensation, and ocular discomfort. Less frequently reported effects include increased intraocular pressure (which may be reversible upon discontinuation), posterior subcapsular cataract formation, delayed wound healing, ptosis, mydriasis, and conjunctival erythema. Rare systemic effects may include headache, dizziness, and taste perversion. Allergic reactions including rash, pruritus, and facial edema have been reported in fewer than 1% of patients.

Drug interaction

Concurrent therapy with systemic corticosteroids or other topical ophthalmic corticosteroids may increase the potential for adverse effects. Patients taking medications that affect intraocular pressure (such as anticholinergics) should be monitored closely. The preservative benzalkonium chloride may be absorbed by soft contact lenses—patients should remove lenses prior to administration and wait at least 15 minutes before reinsertion. No specific pharmacokinetic interactions with systemic medications have been documented, though caution is advised with any medication that affects steroid metabolism or clearance.

Missed dose

If a dose is missed, administer it as soon as possible. However, if it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to make up for a missed one. Maintain as consistent a dosing schedule as possible to ensure therapeutic steroid levels are maintained for inflammatory control. If multiple doses are missed, contact the prescribing physician for guidance on resumption of therapy.

Overdose

Acute overdose with topical ophthalmic corticosteroids is unlikely due to limited systemic absorption. However, excessive application may increase the frequency and severity of local adverse effects such as elevated intraocular pressure, conjunctival hyperemia, and discomfort. In case of accidental ingestion, symptomatic and supportive care should be provided. Dialysis is not effective for corticosteroid removal. Medical attention should be sought if significant quantities are ingested, particularly in pediatric patients.

Storage

Store at controlled room temperature 15°-30°C (59°-86°F). Protect from light and freezing. Keep container tightly closed when not in use. Do not use if solution changes color or becomes cloudy. Discard multi-dose bottles 4 weeks after opening. Single-use containers should be used immediately after opening and any unused portion discarded. Keep out of reach of children and pets.
